I have an Apple Design Keyboard, a new Mac, and cannot start up with extensions off (shift key) or boot from a CD.

Disconnect the Apple USB keyboard or use the Apple USB keyboard for these functions. The iMate does provide for boot keyboard commands such as depressing the Shift key for disabling extensions or C key for booting from a CD. However you can only do this from one of the attached keyboards. When both the iMate (which emulates a keyboard in boot mode) and the Apple keyboard are connected, the Apple keyboard takes precedence during bootup. As an interesting note, if you connect two Apple USB keyboards at the same time only one of them will be able to perform these boot functions.
